How are Illustrators paid?

This is a very elementary question, but I don't know the answer.

Who pays the illustrator? The author? The publisher?

I know that authors pay for photographic art in say a non-fiction book for adults, but what about original art for a middle grade novel?

The publisher hires and pay the illustrator except in self published works.

I don't think that is always the case either. If there is a Publisher involved, they will very often select the photographer and pay them. It would likely depend on the budget the publisher has allowed for the book and who the Author is (a celeb cook book for example)
Perhaps that isn't the sort of non-fiction you had in mind though.

As a general rule, the money should go to the author not away. So if a publisher wants you to select and pay an illustrator, be wary. I do know there are now "co-publishing" arrangements where the author pays for the illustrator and editing end of things and the publisher (cough) pays for printing and distribution. But in my opinion these arrangements aren't much better than vanity publishing.
