I'm writing a Middle Grade with sketches, similar to Dork Diaries, or Diary of a Wimpy Kid. I can't even draw stick figures though. Do I add a box for the illustrations, include only my text, or add an * with notes on the illustrations I picture.

I'd use illustration notes the way you would for a picture book. ( Note: blah. blah.) But do know that this will be a harder sell because it will be harder to convey your vision without the art. Also, make sure those illustrations are carrying part of the story. If they don't add to the story, leave them out. A lot of middle grade is illustrated normally.
